Agreement Forms

The Online Academy PBS staff have created a series of agreement forms for organizing module usage and dissemination efforts.  It is critical that organizations intending to download the PBS modules sign and return a Server Agreement.  Likewise, instructors and students are required to sign and return the appropriate agreement forms below.  If you have questions regarding these agreements, please contact the staff listed below.  All completed and signed contracts should be returned to Dr. Rachel Freeman at her address listed at the bottom of this page.
